The practice adheres to strict rules for patient confidentiality and this will not change under the new GDPR rules.
Any request to access medical records, e.g. immunisation history, test results, hospital letters need to be made to the practice via the form attached, sent by email to enquiries.w97058@wales.nhs.uk or in person at the surgery.
