Sick certificates

You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.

If you have consulted with a GP at the practice and discussed an ongoing sickness absence, a request for a certificate can be sent to the patient services team to process.

Fit Note/ Med3 Request/ Sick Note

This form should only be used to request a certificate where a patient has consulted with the practice regarding their condition or for a continuing absence, which the GP is aware of.

  • DD slash MM slash YYYY
  • This should be the date from either the end of your self certified absence (7 days from onset of illness) or the due date of your continuing certificate. for example, if you were previously signed off until 01.07.2021 the date to start should be 02.07.2021
    DD slash MM slash YYYY
  • Please be aware the length of time the GP certifies may differ from your request.
  • Court Road Surgery aims to utilise text messages for a number of purposes including, but not limited to: Notification of cancellation of appointments, booking appointments, test results, forms ready for collection, invitations to reviews or clinics. It will be the responsibility of patients to ensure that telephone contact numbers are kept up to date.
