We are changing our approach to annual reviews. We will now be:

  • Inviting patients to attend annual review during their birthday month
  • The initial part of your review will comprise of a patient online questionnaire, which can be completed online here, by selecting the appropriate form as directed by the practice team.

Upon receipt of the completed online questionnaire a Clinician will determine one of three outcomes – detailed at the end of the survey.
