From 25th May 2018 new Regulations were introduced, called GDPR – General Data Protection Regulations. In line with this legislation the practice has produced information for patients.
The practice adheres to strict rules for patient confidentiality and this will not change under the new GDPR rules.
Any request to access medical records, e.g. immunisation history, test results, hospital letters need to be made to the practice via the form attached, sent by email to firstname.lastname@example.org or in person at the surgery.
