How to make an appointment for a Flu Vaccine 2023.

We are trying to improve the way that we are booking Flu Vaccines this year so that we can reduce pressure on our team in the practice.

For children’s nasal flu vaccines, children aged 2 and 3 on 31.8.2023 will be invited by the practice in due course.  All school children, including years reception through to secondary school children in year 11, will be vaccinated at school, by the School Nursing Team.

Flu Vaccine appointments for adults can be requested via this online form.   We will text patients with an appointment for the clinics when appointments “go live”.  To reserve your vaccine, please complete the form and we’ll be in touch!

  • Court Road Surgery aims to utilise text messages for a number of purposes including, but not limited to: Notification of cancellation of appointments, booking appointments, test results, forms ready for collection, invitations to reviews or clinics. It will be the responsibility of patients to ensure that telephone contact numbers are kept up to date. By completing this form you are consenting to us contacting you by text.

    If you have multiple medical conditions, such as Diabetic and Asthmatic, you only need to tell us one.
  • Please let us know if you are unable to attend the practice because you are housebound. Housebound is for patients who are unable to leave their home at all. Housebound patients will be contacted by the practice when the Flu@home Team are available to visit.
  • Please indicate your preference of date.
  • The practice is able to provide Covid19 Booster vaccines only to patients aged 65 - 74 years who are NOT housebound. Please indicate below if you wish to have a COVID19 Booster confirming you are aged 65 - 74 and NOT Housebound.
  • Vaccination for Shingles is changing in Wales – and across the UK, so that more people are eligible for the vaccine. The shingles vaccine will be offered in a phased approach to everyone in Wales aged 60 years old until their 80 birthday. From 01.09.2023 we can offer shingles vaccination to: Patients aged 50+ who are severely immunosuppressed (have a very weakened immune system because of a health condition or medical treatment) Patients aged 65 after 01.09.2023 (but not 66 - 69) Patients aged 70 - 79 after 01.09.2023 (but not aged 80+) Shingles is caused by the varicella zoster virus, the same virus which causes chicken pox. Shingles is caused by a reactivation of an infection of a nerve and the skin around it which can be very painful. Most people fully recover from shingles, but for some the pain can last for months or even years after. This is called post-herpetic neuralgia (PHN), which is difficult to treat and can be extremely painful. The older you are, the more likely you are to have PHN as a side effect of a shingles infection and this is an additional reason why the vaccine is offered to older adults.
